2007 health risk assessment resultsThank you for participating in Associated's 2007 health risk assessment and online questionnaire. You've taken a step to obtain health measurements you can use to improve your health and prevent illness and disease. To access the confidential results of your 2007 health risk assessment laboratory draw, click here. You will go to St. John's Web site. Enter the login and password you created when you completed the online health questionnaire. You will not need the AECI group key or wellness identifier. Once you’ve entered the site, click on “lifestyle HRA” under “health risk assessment” on the left side of the screen. This takes you to a new screen. At the bottom of the screen, you'll see an icon that looks like a clipboard. Click on the “Health lab results (lab reported)” icon for your results. You may print out your results for your records and to share with your physician. If a section is marked red, you can click on it to read more about that topic in St. John's book “Healthy Living: A New Start.” Employees also received a copy of this book at their health fairs. Consult your physician about any areas of concern. Online resources include MyHealth, found at www.cooperative.com under “My benefits.” MyHealth also provides health coaches for consulting on conditions and treatments (not for emergency situations though).
